Requirements (all required):
Required qualifications relevant to your modality
Current Professional Indemnity, Public & Products Liability Insurance
Current CPR and First Aid certification
Relevant professional association membership
Licensing agreement signed and returned (supplied by Nxt Level)
Four weeks rent initially (for rental option)
Commitment to maintaining a clean and professional space
